1/3/2023 EnchantaVaughn Approved Behavior: The client was appropriately dressed upon the QMHP’s arrival wearing clean clothes with a fresh haircut. Client greeted his counselor with respect and spoke in a positive tone stating that he was happy to see her. The client’s behavior was acceptable as he was respectful and cooperated with the counselor when prompted to begin his therapy work. Client was in good spirits and happy, as evidenced by his engagement in the conversation, as he reflected on his day and contributed to each intervention. His mother was relaxed and happy that client was able to express how he feels and open up more about his life. Client shared memories and funny stories, as he continued to show respect to his mom and counselor. Mom applauded client for being nice to his counselor and speaking his truth. Client verbalized positive communication in the session and shared all of his ideas with her. Client continued to demonstrate responsible behaviors around the house by cleaning up trash and helping his mom with different tasks. Behavior: Client exhibits a calm state of mind and is monitored with transitioning well between the session and his home duties. Client presented with a normal mood and affect. Client reported that he is feeling good today and he also says that he's having a good day. Client displays positive characteristics of his mental illness by controlling his impulses and considering QMHP directives. Client shared how much he has been learning and what he enjoys best about having an in-home counselor. Client was happy to have his counselor involved with him each week because she always makes him feel important and respected. Client stated that having a counselor was cool and fun. Client displayed good interaction and contributing his thoughts to the one on one conversation. Client has implemented interventions and strategies taught by QMHP in his activities. Client was supported for acknowledging his flaws, as evidenced by him remaining in agreement presenting positive and pleasant behaviors. Intervention: QMHP went to the home for the purpose of meeting with client for individual session. QMHP processed with client about his day. QMHP asked client how he was doing in school and if he was passing his classes. QMHP engaged in conversation with client about his goals and how he plans to achieve them. QMHP discussed with him key factors in his development and mental health skill building so that he understands how he can contribute to his successes while at home. QMHP praised client for his positive behaviors and verbalized to client that he is doing an amazing job with following through with orders and maintaining a good attitude. QMHP encouraged client to keep up the good work and continue to listen to his mom and dad. QMHP acknowledged client for listening and paying attention during the session. Intervention: QMHP spoke with mom in reference to her son’s treatment plan and advised her of any changes and upcoming assessments. QMHP provided parent with additional resources on client’s diagnosis and information that maybe useful for him at home. QMHP discussed various strategies with client to assist him with making progress towards his goals daily. QMHP administered the session to enhance client’s ability to find a hobby or learn a new skill so that he is able to remain active and implement appropriate social behaviors. QMHP praised client for the feedback during the session and showing more effort to contribute to discussion. QMHP challenged client to continue displaying a positive attitude while employing positive reinforcement when client is able to achieve his goals and objectives. Response: QMHP monitored individualized behaviors for acquisition of objectives. Client was open and honest about his feelings, verbalizing encouraging affirmations. Client is making progress with expressing himself. Client made insightful comments and demonstrated good eye contact and positive facial expressions. Client reported that he is kind and polite to his brother and sister who looks up to him as the oldest. Client stated he wants to make them proud. Client appreciates his family for supporting him with all his needs and is thankful for the people that love and care about him. Client was able to demonstrate the correct emotions while presenting appropriate manners towards the QMHP. Client displayed active listening skills during the session, as he nodded his head and responded back to feedback to assure the counselor knew he was engaged in the discussion. Response: Client was receptive to the intervention as he was able to provide a response during the session. Client has improved with staying calm and utilizing relaxation skills in the home. Client responds well to praise and support. Client communicated that he has been implementing acquired skills to deal with his stress and anxiety. Client promised to stay focused and work on himself one day at a time. Client said he was going to practice coping strategies and relaxation techniques in all aspects of his daily living routine in hopes that he will be a better person. Client said he was feeling good and plans to have a good rest of his day. Client was compliant and content with today’s session. His mom thanked QMHP for her patience and service with wanting to help her son reach his fullest potential.
